Waiver Agreement

You must be at least 18 years old to sign a liability waiver. If you are not 18 or older, your parent or legal guardian must sign this waiver on your behalf.

I acknowledge and agree as follows:

• I am aware that the activity may require physical exertion and moving around within a confined space and result in injury.
• The use of any virtual reality equipment entails known and unanticipated risks that could result in physical or emotional injury including, but not limited to dizziness, vertigo, lack of balance, or nausea.
• I accept and assume all of the risks associated with this activity and elect to participate at my own risk with prior knowledge of the dangers.
• I do not have epilepsy and am not prone to seizures or any illness, ailment, or incapacity that may affect participation in the activity.
• I agree to obey instructions and rules and acknowledge that Entermission Melbourne has the right to stop the activity at any time.
• I accept liability for the cost of any damage to the equipment or venue caused by me.
• Entermission Melbourne is not responsible for lost or stolen property.
• Entermission Melbourne may send me emails from time to time, which I can choose to unsubscribe. 
• I acknowledge and AGREE that photos and videos may be taken, uploaded, and may be used for marketing purposes.
